  1. Verify account status and role assignment.
  2. Recheck device time sync for authenticator app.
  3. Retry from a standard browser profile with cookies enabled.
  4. If still blocked, reset MFA enrollment through admin procedure.

  • Capture timestamp, user email, and tenant context.
  • Include route response codes from browser network panel.
